The enhanced VITAS Healthcare mobile app combines hospice referrals, eligibility tools, and educational resources into one secure platform. Healthcare professionals can use the app to submit hospice referrals directly from their mobile devices and track referral status in real time.
The app also includes interactive eligibility tools to help clinicians determine when hospice care may be appropriate. By simplifying the referral process, VITAS aims to improve communication and help patients access care sooner.
The company designed the app for physicians, nurses, social workers, discharge planners, and other members of the care team who regularly coordinate hospice services.
Educational Resources for Healthcare Professionals and Families
In addition to referral tools, the app offers a wide range of educational resources. Users can watch short videos created by hospice experts, read articles about end-of-life care, and join free webinars covering clinical best practices and common care scenarios.
